The Therapy of Shock: Some Lessons from Military Surgery.
نویسنده
چکیده
The state of peripheral circulatory failure, referred to as "shock," is so frequentlv associated with the extensive injuries incurred during military operations, it may properly be called a malady of warfare. Not only is it encountered more frequently, but its severity far exceeds that ordinarily seen in civilian practice. It is thus possible to draw from the experiences of military surgery many points of practical value in the treatment of shock. Because the term "shock" has been used quite loosely to indicate any form of peripheral circulatory collapse, this discussion will be prefaced by a brief definition and classification. Shock is defined as a "state of actual, or impending, peripheral circulatory failure due to a reduction of the effective blood volume not primarily cardiac in origin." This excludes from discussion all forms of peripheral circulatory collapse in which failure of the heart is the primary cause. Depending upon the mechanisms involved, three types of shock may be recognised: neurogenic, a state of sudden loss of vasomotor tone with a redistribution and pooling of blood in dependent portions of the body; vasogenic, a consequence of direct injury to capillaries or small vessels; and hematogenic, the result of a primary loss of fluid from the circulation.4 The term "decompensated shock" is used to indicate the state of -advanced collapse which follows a prolonged reduction of the effective blood volume, irrespective of the original cause or causes.

Background and Aim: Medial Tibial Stress Syndrome (MTSS) is one of the most common sports-related overuse injuries among runners and military personnel. Extracorporeal shock wave therapy (ESWT) is an emerging treatment modality that used for individuals with MTSS. However, there was no found a systematic review that specifically assess the clinical and functional efficacy of ESWT in individuals...

The utilization of negative pressure for medicinal purposes dates back to 600 bc. The U.S. military has been engaged in continuous overseas combat operations since 2001. Negative-pressure wound therapy has been in use in the treatment of casualties from these operations since 2004. It represents a new standard of practice in combat wound care; it promotes granulation tissue formation and create...

Wedelolactone is known to have biological activities such as anti-inflammation hepatitis, anti-hepatotoxic activity, and trypsin inhibitory effect. However, up to date, there has not been studied deeply in the role of wedelolactone for zymosan-induced signaling pathways in the process of regulating the excessive inflammatory responses in host. Here, we demonstrated that wedelolactone plays an e...

“Those who cannot remember the past are condemned to repeat it” [1]. The famous quote from Hispanic American philosopher George Santayana reminds us of the critical importance of constantly reflecting on the most important lessons garnered from both our own personal experiences and those of our peers. In 49 years of academic orthopedic practice, I have frequently reflected on the most important...
متن کاملImplantation Metastasis of Synovial Cell Sarcoma: Lessons Learned from a Mistake
Implantation metastasis occurs when tumor seeds into a wound or tissue by a significant amount of viable tumor cells. Here we describe the case of a 30-year-old man suffering from pain and swelling of the foot, which was misdiagnosed as a bone cyst. Surgery was performed involving curettage followed by bone grafting. After surgical pathology, the exact diagnosis was revealed as the synovial sar...
